THE CHE PROGRAM

OCCHD Cares

The Community Health Engagement program works collaboratively with residents, community partners, and schools, to identify the needs most important to residents. CHE utilizes Community Health Workers (CHWs) to pursue meaningful strategies to address those needs through resource connection, referrals, healthcare navigation, Social Determinants of Health Screenings, and outreach.

Common Services Offered:

  • Assistance with food insecurity
  • Patient navigation
  • Housing assistance
  • School based referrals (such as uniform assistance, absenteeism follow up, etc)
  • Behavioral Health Care navigation
  • Sooner Care application assistance

Mobile Market

Mobile Market is an OCCHD program housed within Community Health Engagement.  The goal of mobile market is to increase and improve access to fresh fruits and vegetables in areas that could be considered food deserts or are structurally disadvantaged and under-resourced. Increasing access and consumption to fresh produce will lead to better health outcomes among Oklahoma City County residents.  There are no income requirements and no I.D. is needed to receive produce from a mobile market stop.
